Unique Smiles are organizing the Summer Camp 2018.

Fill in the below application form to apply for the summer camp or call at 22318404 until the 10^th of June 2018.

Notes:

1. The Summer Camp is available only for families with at least one parent and children.
2. Participation stands for Unique Smiles members only and the yearly fee for 2018 must be settled.
3. The cost of participation for each family is €150 while the rest will be covered by Unique Smiles. In the case of 2 membered families the cost is €100.
